Covered California Reaches Milestone Amid Uncertainty Over Health Subsidies

In a significant milestone for healthcare access in California, Covered California, the state’s health insurance marketplace, has reached a record 1.8 million enrollees—a figure that may continue to grow ahead of the fast-approaching January 31 open enrollment deadline. This surge is largely attributed to enhanced federal subsidies introduced during the COVID-19 pandemic, which have made health insurance plans more affordable for a broader segment of the population.
However, this progress comes with a cloud of uncertainty. The political landscape is shifting as the Trump administration and a Republican Congress prepare to take office, both of which have historically been critical of the Affordable Care Act (ACA), also known as Obamacare. The key concern for Covered California officials is the potential expiration of additional subsidies that have significantly reduced premiums, especially for middle-class families purchasing insurance through the state exchange.
Jessica Altman, Executive Director of Covered California, expressed the urgency of the situation, stating, “Whether there will be action to extend the enhanced subsidies—that’s a big impact that we are closely tracking.” Without an extension, researchers predict that premiums could spike by an average of $967 annually beginning in 2026, potentially leaving around 69,000 Californians without coverage.
In an effort to ensure affordability, California has implemented measures to eliminate deductibles and reduce out-of-pocket costs for mid-tier “silver” plans. Nevertheless, challenges loom on the horizon as potential cuts to Medicaid—known as Medi-Cal in the state—could further strain healthcare funding.
Among the enrollees, approximately 144,000 are newcomers, with nearly 90% qualifying for financial assistance under the program. Residents in Los Angeles and Ventura counties have been granted a special extension for enrollment due to recent wildfires, highlighting the state’s commitment to providing ongoing support during challenging times.
