They don’t call anyone. They pick up their phone, type the symptom and the drug name into a forum, and wait to see if anyone else has felt it too.

Usually, someone has.

A team at the University of Pennsylvania decided to read those conversations. Not a few of them. More than four hundred thousand. What they found adds real questions to the list of Ozempic side effects people search for every day, and it also says something quieter about where people take the things that worry them.

What the researchers actually did

The study, published in the journal Nature Health this year, looked at 410,198 Reddit posts written between May 2019 and June 2025. Every post mentioned semaglutide (sold as Ozempic, Wegovy and Rybelsus) or tirzepatide (sold as Mounjaro and Zepbound).

Nobody could read that much by hand. So the team used large language models, the same kind of AI behind today’s chatbots, to sort what people wrote into the standard medical vocabulary regulators use to log drug reactions.

Of the posts, 67,008 users said they were taking one of these drugs themselves. Of those, 43.5 percent described at least one side effect.

If you have been told these medicines are simple, that number is worth sitting with. Close to half of the people talking about their own treatment were also talking about something it did to them.

The Ozempic side effects everyone expected

Most of what turned up was already known. Stomach trouble led the list by a wide margin: nausea first, then fatigue, vomiting, constipation and diarrhea.

That part matters more than it looks. The researchers treated it as a check on their own method. As senior author Sharath Chandra Guntuku put it, finding well-known effects like nausea “shows that the method is picking up a real signal.” If the AI had missed the obvious, nobody should trust what it found beyond it.

It didn’t miss the obvious. So the next part deserves attention.

The side effects nobody had written down

Two groups of complaints kept showing up that are not well captured in the drugs’ labels or in the original trials:

Changes to the menstrual cycle and other reproductive symptoms. Nearly 4 percent of the users who reported side effects described something in this group.

Temperature problems: chills, hot flashes, and simply feeling unusually cold.

Here is the detail most summaries skip. The Reddit crowd the team studied leans younger and more male than the general public. The researchers said so themselves. Menstrual symptoms still surfaced clearly in a group like that.

And there is a possible reason they might be real. Jena Shaw Tronieri, a researcher at Penn’s Center for Weight and Eating Disorders, explained that these drugs are thought to work through the hypothalamus, a small part of the brain that “helps regulate a wide variety of hormones.” The same region is involved in body temperature and reproductive hormones. That is why the team thinks these reports deserve a closer look. (If you want to know more about what these drugs do inside the brain, we covered how Ozempic works in the brain and why scientists had part of it backwards.)

What this study cannot tell you

This is the part to read twice if you take one of these medicines.

The study does not show that the drugs cause these symptoms. First author Neil Sehgal said it plainly: “We can’t say that GLP-1s are actually causing these symptoms.” People on these drugs are often losing weight fast, changing how they eat, and dealing with other health conditions. Any of those could play a part.

Forum posts are also not medical records. Nobody checked who wrote them or confirmed their dose. And, as the team pointed out, Reddit users are not a fair sample of everyone taking these drugs around the world. They skew younger, more male, and more American.

What the researchers are claiming is smaller and more useful. In Sehgal’s words, it is “a signal worth investigating.” They plan to look beyond Reddit and beyond English-language communities next.

Why people told a forum first

Clinical trials are built to catch the dangerous things. They are careful and they are slow. Guntuku called them “the gold standard” but noted that, by design, they take time.

Meanwhile, millions of people are living with these drugs right now. Co-author Lyle Ungar described online patient groups as working “a lot like a neighborhood grapevine,” with people swapping notes in real time. The Penn team’s point was that much of this rarely reaches a doctor’s appointment or an official report.

It is easy to understand why. An appointment is short. A hot flash at 2 a.m. feels too small to mention, or a little embarrassing, or you can’t remember it by the time you’re sitting in the chair. There can also be shame attached to the medicine itself. People on these drugs often get judged, which we looked at in why Ozempic users face more social judgment. So people tell strangers who won’t look at them sideways.

The forum listens. But the forum can’t examine you.

There is a very old story about exactly this kind of person. A woman had been sick for years. She had seen doctor after doctor and spent everything she had, and she only got worse. So she didn’t ask anyone for anything. She slipped through a crowd, touched the edge of Jesus’ cloak from behind, and planned to disappear.

He wouldn’t let her. He stopped the whole crowd and asked who had touched Him. His friends thought the question was silly, since people were pressing in from every side. But He kept looking until she came forward, shaking, and told Him the whole story. Only then did He send her home in peace.

The healing had already happened. What He stopped for was the telling. Being heard, fully and without hurry, was part of what she needed. It is one of the oldest pictures we have of a God who isn’t too busy for the symptom you think is too small to mention.

If you take one of these drugs, what to do with this

Don’t stop your medicine because of this study. It found questions, not answers. Talk to whoever prescribed it first.

Keep a simple note on your phone. Write the date, what you felt, and how long it lasted. Chills, cycle changes, tiredness, anything. It costs nothing and it turns a vague memory into something a professional can use.

Say the small things out loud. If a visit feels rushed, lead with your note. A pharmacist can also be a good first person to ask, and in many places it costs nothing.

Use forums for company, not diagnosis. It really does help to know you’re not the only one. Just let that be the start of the conversation, not the end of it.

If worry about your body is keeping you up, it may help to check in with yourself honestly. Our free How Anxious Am I? assessment takes a few minutes.

The part that stays with you

Four hundred thousand posts is a lot of people who felt something and wanted someone to hear it. The researchers found real value in those words, enough to send scientists back to look again.

That might be the best takeaway, whether you take these drugs or not. What you notice in your own body is worth saying. The small, strange, slightly embarrassing symptom is worth saying. Say it to someone who can actually do something with it, and don’t let a hurried room convince you it was too minor to mention.

Discussion Question

Should doctors and drug regulators treat what patients say in online forums as real evidence, or is it too unreliable to act on? Where would you draw the line? The part that got me was why so much of it goes to strangers instead of doctors. https://bgodinspired.com/?p=115440

Questions People Ask

What side effects of Ozempic were found in Reddit posts?

A University of Pennsylvania study published in Nature Health in 2026 analyzed 410,198 Reddit posts about semaglutide (Ozempic, Wegovy, Rybelsus) and tirzepatide (Mounjaro, Zepbound). The most common complaints were nausea, fatigue, vomiting, constipation and diarrhea. The researchers also found reproductive symptoms, such as menstrual irregularities, and temperature complaints, such as chills and hot flashes, that are not well captured in current drug labels or trials.

Does Ozempic cause chills or hot flashes?

It has not been proven. People taking semaglutide and tirzepatide reported chills, hot flashes and feeling unusually cold in online posts, and University of Pennsylvania researchers flagged these as potential side effects worth investigating. The study’s authors stated that they cannot say the drugs actually cause these symptoms, because forum posts cannot rule out other causes like rapid weight loss or other health conditions.

Can GLP-1 drugs affect the menstrual cycle?

Researchers have flagged it as a possibility. In a 2026 University of Pennsylvania analysis of Reddit posts, nearly 4 percent of users who reported side effects from semaglutide or tirzepatide described reproductive symptoms such as menstrual changes. The drugs are thought to act on the hypothalamus, a brain region that helps regulate many hormones, which is one reason researchers think these reports deserve further study. Anyone noticing cycle changes should tell their prescriber.

How many people on Ozempic or Mounjaro report side effects?

In a University of Pennsylvania study of Reddit posts from May 2019 to June 2025, 67,008 users said they were taking semaglutide or tirzepatide, and 43.5 percent of them described at least one side effect. Because Reddit users skew younger, more male and more US-based, this figure describes that online group, not every person taking these medicines.

Should I stop taking a GLP-1 medication if I have unusual symptoms?

Do not stop a prescribed GLP-1 medication such as Ozempic, Wegovy, Mounjaro or Zepbound without first talking to the prescriber or a pharmacist. Write down what the symptom is, when it started and how long it lasts, then share that note at the next contact. Seek urgent medical care for severe symptoms such as intense abdominal pain, persistent vomiting or signs of an allergic reaction.

Source: Sehgal, Guntuku, Ungar and Tronieri, “Self-reported side effects of semaglutide and tirzepatide in online communities,” Nature Health (2026).
